Before your visit
Regardless of which office your appointment is scheduled for, we ask that you be considerate with the following requests:
Arrive 20 minutes BEFORE your scheduled appointment time.
Please be prepared to pay your insurance co-payment upon arrival. Should you have any questions about our Financial Policy below, please contact us.
You should have received a packet of information and forms in the mail. Please complete and fax back to us at this number (502) 583-8001 prior to your appointment. This allows us to get you set up in our new Electronic Medical Records system and save you time when you arrive.
Payment is due at the time services are rendered. You may pay by cash, check, Visa or Mastercard.
We will file insurance for covered services for all plans with which we participate. We do not participate with all plans. It is the patient’s responsibility to check with their insurance company to ensure the physician they are seeing is participating. Please contact your insurance carrier for benefit information and to determine whether services in our offices will be covered.
If your insurance carrier requires that you have a referral, you will need to obtain this referral from your primary care physician in order for us to see you. Both the Kentucky Passport program and some of the Hoosier Healthwise programs require referrals from primary care physicians. Please check with your insurance carrier to determine if a referral is needed for you to see us.
If there is a change in your insurance coverage or benefits, it is your responsibility to notify our office as soon as possible.
We do participate with Medicare and will file your Medicare claims for you. Our office services are paid under Medicare Part B. Please let us know if you have a policy to supplement your Medicare benefits. We will be happy to file a claim with this carrier if Medicare does not automatically submit this for you.
Any balance on your account not paid by insurance within 90 days will become your responsibility and payment will be due from you. We do all we can to provide pertinent medical information on your claim, however we are unable to act as an intermediary between you and your insurance carrier. Please contact you insurance customer service representative if you are dissatisfied with any claim denial and feel your service should be covered.
